  1. Procedures for responding to requests for disclosure, etc.

  1. (3) After receiving your request by e-mail, we will send you our Personal Information Disclosure Request Form. In principle, we will reply to your request by e-mail.
    (4) In order to confirm that the person making the request for disclosure, etc. is the person himself/herself or his/her representative, it is also necessary to submit either ① or ② of the following certificates. The Company will check whether the submitted certificate matches the information on the Personal Information Disclosure Request Form and the relevant personal information subject to disclosure held by the Company
    ① Driver’s license, passport, physical disability certificate, resident card, special permanent resident certificate, alien registration certificate, basic resident registration card, my number card (front side only; do not submit the back side) For public offices and special corporations, submit a copy of an official ID card, such as an issued ID card with a photograph.
    *Official ID cards must be valid within the period of validity.② Submission of a copy of two of the following: health insurance card, resident certificate, pension book, driving record certificate, various pension certificates, bill for payment of public charges, and student book.
    (5) The Company will charge for the actual expenses incurred in the procedures for notification of purpose of use and disclosure (such as expenses required for investigation and confirmation) after obtaining prior consent.
    (6) The Company may not be able to respond to requests for disclosure, etc. in any of the following cases.
    ① When the requestor cannot be identified
    ② The disclosure request documents are incomplete.
    ③ When the personal information in question does not exist in the data held by the Company
    ④ When there is a risk of harm to the life, body, property, or other rights and interests of the requestor or a third party
    ⑤ When there is a risk of significant hindrance to the proper execution of the Company’s business
    ⑥ In case of violation of laws and regulations

    The Company will not provide retained personal data to any third party without prior consent, except in the following cases
    ① When personal data is provided to a third party in accordance with other laws and regulations other than the Personal Information Protection Law
    ② When the provision of personal data to a third party is necessary for the protection of the life, body, or property of an individual, and it is difficult to obtain the consent of the individual.
    ③When it is particularly necessary to provide personal data to a third party in order to improve public health or promote the sound growth of children, and it is difficult to obtain the consent of the individual.
